Superbill Available Upon Reques

While we do not accept insurance as an out-of-network provider, a superbill can be provided upon request. This document can be submitted to your insurance company for potential reimbursement.

(Please note, Medicare and AHCCCS plans are not reimbursable, and reimbursement for other plans is not guaranteed as it depends on your specific insurance policy).

Psychiatry Sessions (MD, DO)

Initial Comprehensive Psychiatric Evaluation with Psychiatrist $495

This 90-minute appointment with our psychiatrist includes a detailed and thorough assessment, resulting in a diagnosis and a personalized treatment plan. Beginning your care with this comprehensive evaluation is crucial, as it allows us to understand your unique needs from the very start, setting the stage for the most effective treatment. To ensure the best care, screening and history forms are required at least 48 hours before your appointment. Our mission is to help our patients uncover the root causes of their struggles, which may require medical tests such as laboratory work, EKG, or imaging (in-office labs and EKG coming soon).

A regular comprehensive evaluation is required as the first step in order to avoid misdiagnosis and to identify any comorbidities. Because of this, we do not offer free consultations. However, you can request a 25-minutes consultation for $165 by calling our office.

In some cases, additional screening or testing may be necessary to confirm a diagnosis.

Autism Evaluation $495

If a suspicion of Autism Spectrum Disorder arises during the initial evaluation or ongoing treatment, an additional 60-minute appointment may be scheduled. This session will review clinical evaluations, parent reports, collateral information, and questionnaires. It includes a psychiatric evaluation report, required documentation, and clear instructions for parents to submit to the Developmental Disabilities Division to begin the process of seeking services.

25 Minute Follow-Up with Psychiatrist $165

This session focuses on medication management or behavioral concerns in children. It is not recommended for teenagers and is not intended for therapy

50 Minute Follow-Up with Psychiatrist $305

Ideal for teenagers, this session can include therapy with the psychiatrist, in combination with medication management if applicable.

Therapy Sessions

These appointments can be used for various therapeutic approaches, including EMDR, Cognitive Behavioral Therapy (CBT), Exposure and Response Prevention (ERP), and Dialectical Behavior Therapy (DBT). Weekly sessions are recommended, especially for complex cases.

Appointments with Behavioral Health Technician (BHT)

For patients participating in therapy with a Behavioral Health Technician (BHT), this session may include Exposure and Response Prevention (ERP), Cognitive Behavioral Therapy (CBT), Mind-Body Medicine, Dialectical behaviour therapy (DBT), social skills training, and more.

Meet our BHT therapy providers

Ketamine Infusion Therapy with Nurse $399

For established patients only with treatment-resistant depression who are deemed good candidates. Typically, these ketamine infusions* are usually recommended in a 6 session course, however we usually review patient response and determine if appropriate to continue with remaining sessions. Transcranial Magnetic Stimulation (TMS)
