No Show & Cancellation Policies

Initial Intake Appointments

  • For initial intake appointments, you must provide at least 48 hours notice for cancellations or rescheduling.

  • Failure to do so, including no-shows, will result in a charge of the full appointment fee ($495 for the Initial Comprehensive Evaluation).

No-show and cancellation policy:

  • For all follow-up appointments, if rescheduling, cancellations or no-shows occur within 24 business hours of the appointment time, the patient will be charged at 100% of the appointment fee. We understand that there are difficult circumstances like sickness, traffic and other unexpected problems that arise, however, the practice cannot operate and provide the level of care patients want without this policy in place. Virtual or phone appointments may be available if needed.

Payment and Insurance Policies

  • All appointments require a card on file, which will be automatically charged at the time of service.

  • We do not accept insurance but will keep your insurance information on file (read more about our Transparent Pricing and Payment Solutions).

Additional Policies

  • We cannot see patients with a sex offender status.

  • Controlled substances may not be continued, and treatment is determined on a case-by-case basis.
